We shall be visiting school for the 2nd visit on Tuesday 13th December 2022 to deliver the Nasal Flu spray programme. Thank you to those who have completed the on-line consent form already. If you previously completed a consent form, but your child missed their vaccination due to absence etc. you DO NOT need to complete another form, however if you did not complete a consent form for the original date then please complete one using the guidance below.
